Grace Health is seeking a Telephone Triage Nurse to provide on-site professional telephone nursing care for Grace Health patients. This individual will work Monday - Friday. No weekend or holiday shifts! This position offers the option to work from home.

EXAMPLES OF DUTIES: (This list may not be all inclusive.)

1. Collects the health status of the caller.

2. Uses symptom based protocols to evaluate the needs of the patient.

3. Advises patients about healthcare options.

4. Maintains a log of all calls.

5. Documents assessment, interventions/options offered, the agreed upon plan of care and the protocol used in the medical record.

6. Collaborates with other Grace Health staff regarding patient care.

7. Identifies patients who need referrals to other community agencies.

8. Processes requests for prescription refills and lab results.

9. Processes incoming calls to the Neighborhood.

Knowledge, Skills and Abilities:

  • Knowledge of professional nursing theory and practice to give and evaluate patient care.
  • Knowledge of organizational policies, regulations and procedures to administer patient care.
  • Knowledge of medical equipment and instruments to administer patient care.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
  • Skill in identifying problems and recommending solutions.
  • Skill in preparing and maintaining records, writing reports, and responding to correspondence.
  • Skill in establishing and maintaining effective working relationships with patients, medical staff and the public. Ability to react calmly and effectively in emergency situations.
  • Ability to interpret, adapt, and apply guidelines and procedures.
  • Ability to communicate clearly.

Education: Graduate of an accredited school of nursing.

Experience: Five years of professional nursing experience preferred, including one year medical/surgical experience.

Certificate/License: Possession of a State nursing license.
