Grace Health is looking for a Wellness Nurse to join our growing team! This position will increase patient satisfaction by decreasing waiting time, and making some time available for providers to see acute and chronic patients. No nights/or weekends.

EXAMPLES OF DUTIES: (This list is not all inclusive.)
1. Assessment of patient/family’s health, pain, nutritional, psychosocial, behavioral, cultural, age specific and learning needs.
2. Promotion of preventive health care.
3. Uses principles of learning to participate in education with the patient and family.
4. Advocates for the rights of the patient and family.
5. Safely administers medication and treatments per standard of care.
6. Provides referral to community resources for patients/family.
7. Communicates with patients/family about the plan of care.
8. Anticipates needs of the provider.
9. Participates in professional development activities and maintains professional affiliations.
10. Maintains required licensure and optional certification.
11. Actively provides appropriate medical advice based on the RN skill set.
12. Data entry/documentation of conversion and plan of care in EMR via computer.

PERFORMANCE REQUIREMENTS
Knowledge, Skills and Abilities:
· Knowledge of professional nursing theory and practice to give and evaluate patient care.
· Knowledge of organizational policies, regulations and procedures to administer patient care.
· Knowledge of medical equipment and instruments to administer patient care.
· Knowledge of common safety hazards and precautions to establish a safe work environment.
· Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
· Skill in identifying problems and recommending solutions.
· Skill in preparing and maintaining records, writing reports, and responding to correspondence.
· Skill in establishing and maintaining effective working relationships with patients, medical staff and the public.
· Ability to react calmly and effectively in emergency situations.
· Ability to interpret, adapt, and apply guidelines and procedures.
· Ability to communicate clearly.
Education: Graduate of an accredited school of nursing
Experience: One year of professional nursing experience in a clinic setting preferred
Certificate/License: Possession of a Michigan Registered Nurse license
Current BCLS certification
